This is a form that can be given to high school students and their parents, which enables them to keep the student's contact information, and other information regarding courses of study and extracurricular activities, private form military recruiters. Under the No Child Left Behind Act, this information automatically goes to the recruiters, after the first 30 days of the school year.
The flyer also has information on how to opt out of this information being included in a separate Pentagon database.
